Minors between the ages of 5 – 17 must be accompanied by a parent or guardian and bring with them a completed parental consent form. Individuals 12 years of age and older are eligible to receive the booster dose of the vaccine if they were fully vaccinated at least six months ago. Proof of Florida residency is not a requirement to receive the vaccine in Florida but individuals must provide a photo identification. Currently, there are several ways to get vaccinated from various providers including County, State and Federal vaccination sites and programs. We strongly encourage everyone who is eligible to receive the COVID-19 vaccine to get vaccinated. The vaccine is our best shot at recovery and a return to complete normalcy.
